STEP 1: Complete an online application at New Agency Application
STEP 2: Notify your HMIS Lead via email that you have completed the online application. HMIS Leads are listed below based on the appropriate CoC.
STEP 3: Your local Continuum of Care Lead will be responsible for approving your application.

Once approved for participation, your local CoC Coordinator will request a signed MOU, available below.
Once you receive notification your Agency has been approved, you may immediately sign up your staff for Training. You may only use the Schedule Training tool once your agency has completed the Application.
